Pre-Participation Paperwork

The Geneva College Sports Medicine staff is responsible for your injury prevention, care, and safety while participating in athletics.

In order to provide the appropriate care upon your arrival and during your stay on campus we request your cooperation with the following items. Geneva College Athletic Department requires all student-athletes to enter their personal and health history information into our online electronic medical records database. This is a secure database, used by a selected number of departments across campus including the athletic training staff.

Freshman and Transfers ONLY- Due by 07/20/2025

Paperwork

Please log on to the Geneva College Student Health Portal at geneva.studenthealthportal.com using your Geneva credentials. Athletes are required to fill out the following forms to their entirety which you can find under the ‘My Paperwork’ tab:

1.       Athletic Health History form

2.       Assumption of Risk form

3.       Sickle Cell results upload

a.       Results can be obtained by contacting your birthing hospital or your local state department of health for records.

b.       If records can’t be obtained, you can obtain results by getting a blood test completed with your PCP or another testing organization.

4.       Insurance verification

a.       Clear picture of the front and back of the VALID insurance card

Physical exam

All athletes are required to obtain a physical on their own and upload the signed form on your PyraMed Health portal BEFORE coming to campus. Preferably your physical is completed with your PCP but can be signed off by the appropriate medical provider as instructed on the form. We will NOT accept any other signed physical forms! Impact Testing

Every new athlete at Geneva needs a fresh baseline test to start participation in athletics. This test must be completed on a computer, not a cell phone or tablet.  This test will take approximately 15-20 minutes to complete.  Please devote your attention to this test so that you can complete an accurate baseline assessment.  If it is deemed inadequate, you will be asked to take it again. Instructions:

       1.       Go to Impacttestonline.com/testing

       2.       Type in the following code: c48e86f952. 

       3.       Please fill out the demographics.

       4.       At the end of the test, a box will pop up asking to send a confirmation/verification email for your test.  Please type in and use Madi’s email, mmpresto@geneva.edu to have it sent to her.

 

For Returners ONLY- Due by 07/20/2025

Drug Testing Policy:

Geneva College understands that it is a privilege for a student to represent Geneva College on or off the field and the responsibility of such representation is a matter of both character and integrity. Geneva College, along with the NCAA, seeks to uphold this responsibility by providing programs to insure a fair, safe, healthy and honorable field of play.

Any student-athlete that is currently taking prescription medications for Attention Deficit Disorder, Anxiety, or Depression must have a letter from the prescribing physician ON-FILE in the athletic training room by completing the ADHD Examination Form. The medications normally prescribed for the above conditions can result in a positive NCAA drug test. The information is required by the NCAA to be On-File prior to any drug testing administration as athletic eligibility can be affected by a positive drug test. Please follow the instructions to complete these additional requirements if you meet the above description.
